Registration Form

WHAT:  PHSA sponsors a group of teachers who offer special classes to supplement your home-schooling needs.
Teachers are screened by the support group, but are independent contractors with the families they teach.  A
family must sign waivers of liability for Shadycrest Baptist Church and follow behavior guidelines in order to
participate. Registration and contact is done with each teacher.  Please consider when you sign up for a class that
you are making a year-long commitment to the teacher.

WHEN:  All classes will meet on  Wednesdays.

CLASSES WILL BEGIN AUGUST 27,  2008

2008-2009 Class Days and Holidays:

 August 27 - First Class

September 3 – Tuition Due

October 1 – Tuition Due

Oct. 22 – Fall Break, no classes

November 5 – Tuition Due

Nov ember 26 – Thanksgiving Break, no classes

December 3 – Tuition Due

December 17 – Christmas Break, no classes

 

December 24 – Christmas Break, no classes
 

December 31 – Christmas Break, no classes

 

January 7 – Classes Resume, Tuition Due

 

February 4 – Tuition Due

 

March 4 – Tuition Due

 

March 18 – Spring Break, no classes

 

March 25 – Spring Break/Make-up Day (cancelled classes may make up missed time on this date)

 

April 1 – Tuition Due

 

May 6 – Tuition Due

 

May 20 – Last Class

WHERE: All classes will be offered at  Shadycrest Baptist Church, White and Red Annex, 3007 Yost Road, Pearland.

HOW: To register for a class, call or e-mail the teacher.  If you need answers to general questions about classes, send

Teacher's require a Registration/Lab/Supply Fee to hold a student's place in the class. You may click on the
name of each class to get a more detailed description, including fees and textbooks needed. 

TUITION:  Tuition is due the first Class Day of each month, September through May.  Tuition is the same each month regardless of how many days the class meets.  We guarantee that 31 out of 32 classes will meet or the teacher will schedule a make-up class or a tuition refund. 

A $10 late fee is charged when tuition is late.

Families must pay a Registration fee to PHSA to take a class.  Registration  fee is $10.00 per year, per family.  Registration form for Classes and can be downloaded by clicking here, can be requested from a teacher, or can be mailed to you by contacting Vicki @281-485-9783, e-mail: <PHSApearland@gmail.com>

STUDY HALL: Students who have back-to-back classes or who need to wait for a sibling to finish class are welcome to wait in our designated area/study hall as long as they have something to work on and the teaching staff knows that they are going to be waiting.  If students are disruptive they will lose this privilege.

LUNCH, FOOD & SNACKS: Students are welcome to bring a snack or lunch if their classes run through the lunch hour. Teachers will allow them to have a break to eat their lunch. Please remind your students to clean up after themselves and put trash in the proper place. If students leave messes they will lose this privilege.
